Origin of Guru Brahma Guru Vishnu
Guru Brahma Guru Vishnu verse is taken from Guru Gita, which is said to be part of Visvasaratantra from Uttarakhanda of Skandapurana.
Guru Gita describes a conversation between Shiva and goddess Parvati, in which she asks him how to achieve liberation.
केन मार्गेण भो स्वामिन् देही ब्रह्ममयो भवेत् ।
तत्कृपां कुरु मे स्वामिन् नमामि चरणौ तव ॥
“Lord! Which is the path that will lead the jiva imprisoned in body to the state of Brahman? Please tell me about that. I Salute at your feet.”
Shiva answers by describing the importance of the guru and he can only lead a student to the path of liberation. The whole Uma Maheshwara dialogues are known as Guru Gita.

गुरुर्ब्रह्मा गुरुर्विष्णुः गुरुर्देवो महेश्वरः ।
गुरुः साक्षात् परब्रह्म तस्मै श्री गुरवे नमः ॥
गुरु ब्रह्मा हैं, गुरु विष्णु हैं, गुरु महेश्वर शिव हैं, गुरु ही परब्रह्म परमेश्वर हैं; ऐसे श्रीगुरु के प्रति मेरा नमन है !
